When your face begins to look more tired or drawn, even when you feel well
Facial volume loss can subtly change the way you recognise yourself.
Perhaps your cheeks look flatter than they once did. The area beneath your eyes may appear more hollow, or folds around the mouth seem deeper even though your weight and lifestyle have not changed. After weight loss, hormonal changes or simply the passing of time, your face may look more tired or drawn than you feel.
These changes are not always caused by wrinkles alone. The face is supported by layers of bone, fat, muscle, connective tissue and skin. When those structures change, the contours above them change too.
At Skin Perfection London, we assess whether your concern appears to involve true structural volume loss, changing skin quality, dehydration, loss of firmness or a combination.
Our treatments can improve hydration, texture, collagen support and the appearance of some fine lines. However, they cannot physically replace substantial lost facial volume. We will always make that distinction clear before recommending a plan.

Facial volume loss describes a reduction or redistribution of the structures that give the face shape and support.
These include:
- Facial fat compartments
- Bone structure
- Collagen and elastin
- Muscle
- Connective tissue
- Water and naturally occurring hyaluronic acid within the skin
Volume loss can alter the transition between different areas of the face. A cheek may look flatter, the temples more hollow or the under-eye area more shadowed.
As support changes, the skin may also begin to fold or sit differently. This can make lines around the nose and mouth appear deeper, even when the quality of the skin itself remains relatively healthy.
True structural volume loss is different from dehydration. Dehydrated skin may look temporarily less plump, but it does not mean that facial fat or bone has disappeared.
What does loss of facial volume look like?
The pattern varies between patients. You may notice:
- Flatter or less defined cheeks
- Hollowing beneath the cheekbones
- More visible under-eye hollows
- Shadowing around the eyes
- Hollowing at the temples
- Deeper folds from the nose towards the mouth
- Lines extending downwards from the corners of the mouth
- Reduced fullness around the lips
- A less defined jawline
- A more tired, drawn or angular appearance
- Skin that seems to have less underlying support
- Changes following significant or rapid weight loss
Some changes are caused primarily by lost volume. Others result from skin laxity, muscle movement, pigmentation or shadowing.
This matters because no single treatment can correct every one of these concerns.
What causes facial volume loss?
Facial volume changes usually develop through a combination of natural ageing, genetics, lifestyle and changes in body composition.
Natural ageing
As the face ages, fat compartments can lose volume, change shape or move in relation to the surrounding tissues.
The bones of the face also change gradually, while collagen production declines and existing collagen becomes less organised.
Together, these changes can reduce support beneath the skin and alter facial contours. Research into facial ageing increasingly recognises the importance of fat loss in both superficial and deeper facial tissues.
Weight loss
Losing weight can reduce facial fat as well as body fat.
This may be particularly noticeable after:
- Significant weight loss
- Rapid weight loss
- Illness
- Major lifestyle changes
- Bariatric surgery
- Weight-management medication
The degree of change depends on your age, starting weight, genetics, skin elasticity and how quickly the weight was lost.
Rapid weight loss can sometimes leave the face looking hollow before the skin has had time to adapt.
Hormonal changes
Perimenopause and menopause can affect collagen, hydration, skin thickness and elasticity.
As hormone levels change, the skin may become drier, thinner or less resilient. This can make existing volume changes and facial folds appear more noticeable.
Genetics and natural facial structure
Some people naturally have a leaner or more angular face. Others have fuller cheeks or more prominent facial fat compartments.
The way your relatives age may offer some indication of where you are likely to notice volume changes, although lifestyle and environmental exposure also influence the process.
Sun exposure and smoking
Sun exposure and smoking affect collagen, elasticity and skin quality.
They do not necessarily remove facial fat directly, but they can reduce the skin’s ability to remain firm and supported. This may make underlying volume loss appear more pronounced.
Medical and dental factors
Dental changes, injury and certain medical conditions can sometimes alter facial appearance or symmetry.
New, sudden or one-sided facial hollowing should receive appropriate medical or dental assessment rather than being assumed to be ordinary ageing.
Facial volume loss or dehydration?
It is easy to confuse dehydrated skin with volume loss because both can make the face look tired.
Signs that may suggest dehydration include:
- Fine surface lines
- Tight or uncomfortable skin
- Temporary dullness
- Rough texture
- Makeup settling unevenly
- Skin that improves after appropriate moisturisation
Signs that may suggest structural volume loss include:
- Hollow cheeks or temples
- Persistent under-eye hollows
- Flatter facial contours
- Deeper folds caused by reduced support
- Changes following significant weight loss
- Altered shadows that remain despite good hydration
You can also have both at the same time.
A hydrating facial or skin booster may make dehydrated skin look fresher and more luminous. It will not rebuild a lost cheek fat compartment or replace changes in bone structure.
That difference is central to setting realistic expectations.
Facial volume loss or loose skin?
Volume loss and skin laxity are connected, but they are not identical.
Volume loss means there is less underlying structural fullness or support.
Laxity means the skin and supporting tissues have become looser or less elastic.
A patient may have:
- Volume loss without significant laxity
- Laxity without substantial volume loss
- Both concerns together
- Mainly dehydration or poor skin quality that mimics early laxity
Collagen-stimulating treatments may improve skin texture and create a firmer-looking surface. They cannot reposition descended tissues or remove significant excess skin.
Where substantial laxity is the main concern, surgical treatment may be the only option capable of producing a true lifting result.
Why facial volume loss can make lines look deeper
Skin does not sit independently from the structures beneath it.
When the cheeks or other areas lose support, the skin may fold differently. This can make:
- Nasolabial folds appear deeper
- Lines around the mouth become more noticeable
- Under-eye hollows cast stronger shadows
- The lower face look heavier
- Fine lines appear more pronounced
- The jawline look less defined
Improving the surface quality of the skin may soften some of these changes. However, if the main cause is structural volume loss, a skin treatment alone will have limits.
This is why we would not describe microneedling, laser resurfacing, exosomes or mesotherapy as direct replacements for dermal filler or fat transfer.
What can our treatments realistically improve?
Skin Perfection London does not currently use dermal filler to replace lost facial volume.
Our treatments may instead support the quality of the skin surrounding areas of volume change.
Depending on your suitability, we may help improve:
- Dehydrated-looking skin
- Fine surface lines
- Uneven texture
- Early loss of firmness
- Dullness
- Enlarged-looking pores
- Sun damage
- Uneven pigmentation
- Overall skin quality
- The way light reflects from the complexion
Healthier, smoother and better-hydrated skin can make the face look more refreshed. It will not physically rebuild lost fat, bone or deep structural support.
If replacing volume is your main priority, we will explain that treatments such as dermal filler, fat transfer or surgery would need to be discussed with an appropriately qualified medical practitioner.
Which treatments may support skin affected by volume loss?
Your plan should be based on what you would like to improve. If your main frustration is tired-looking, dehydrated or creased skin rather than the loss of volume itself, the following options may be considered.
Mesotherapy and skin boosters
Mesotherapy introduces carefully selected professional formulations into the skin using multiple small injections.
Depending on the formulation and your suitability, treatment may support:
- Hydration
- Fine texture
- General skin quality
- A brighter-looking complexion
- Early loss of firmness
Some formulations contain hyaluronic acid, but they are not necessarily dermal fillers.
A skin booster designed to improve hydration should not be expected to reconstruct cheeks, fill deep hollows or change facial structure.
Dermapen 4 microneedling
Dermapen 4 creates controlled microchannels in the skin, stimulating a natural repair response and supporting collagen and elastin production.
It may help improve:
- Fine lines
- Uneven texture
- Enlarged-looking pores
- Early loss of firmness
- General skin quality
Microneedling may help the skin appear smoother and firmer, but it does not replace lost facial fat or alter bone structure.
Dermapen 4 with exosomes
For suitable patients, Dermapen 4 may be combined with an exosome-based topical treatment.
This may support skin recovery, texture and overall quality following microneedling.
Exosome treatment should not be presented as a substitute for filler, fat transfer or surgery. It does not physically restore structural facial volume.
Palomar Icon 1540 laser resurfacing
The Palomar Icon 1540 fractional non-ablative laser stimulates repair and collagen remodelling beneath the skin’s surface.
It may help suitable patients improve:
- Fine lines
- Uneven texture
- Early loss of firmness
- Enlarged-looking pores
- Sun-damaged skin
Laser resurfacing may improve the quality of the skin overlying an area of volume loss. It cannot refill the underlying space.
Treatment is not suitable for every skin tone and requires consultation and patch testing.
ThreeForMe
ThreeForMe combines Palomar Icon IPL with fractional 1540 laser treatment.
It may be appropriate when the complexion has several concerns at once, such as:
- Fine lines
- Uneven texture
- Pigmentation
- Sun damage
- Facial redness
The treatment may help the skin appear clearer and more refined. It is not a direct volume-restoration treatment.
DermaFrac
DermaFrac combines gentle microchanneling with the infusion of a selected professional serum.
It may support hydration, brightness and mild surface refinement with relatively little interruption to your routine.
It is most relevant when dehydration or dullness is making facial hollows appear more noticeable.
Professional skincare
A carefully selected routine can improve hydration, barrier function, pigmentation and general skin quality.
Skincare cannot replace structural volume, but it may help prevent dryness and sun damage from making volume loss appear more pronounced.
What treatments genuinely replace lost facial volume?
Treatments that directly restore or redistribute structural volume generally include:
- Dermal fillers
- Facial fat transfer
- Facial implants in selected cases
- Surgical lifting or repositioning procedures
These treatments sit outside the services currently provided at Skin Perfection London.
Dermal fillers can add volume, but they are medical cosmetic procedures with potential risks. Fat transfer and facial surgery involve separate medical and surgical considerations.
If you decide to explore these options, choose an appropriately qualified and experienced medical practitioner who can assess your anatomy, explain the risks and provide suitable follow-up care.
No treatment should be chosen simply because it is fashionable or immediately available.
