How does Palomar Icon 1540 laser skin resurfacing work?

The Palomar Icon 1540 is a fractional, non-ablative laser handpiece.

Fractional means that the laser treats the skin through many precise microscopic columns rather than affecting the entire treatment area at once. These columns deliver controlled heat into the deeper layers of the skin while leaving areas of surrounding tissue untreated.

This controlled response activates the skin’s natural repair process and stimulates collagen remodelling. As new collagen develops and existing tissue reorganises, the treated area can gradually become smoother, firmer and more even.

The handpiece also uses contact cooling to help protect the surface of the skin and improve comfort during treatment.

The depth and intensity of the treatment are selected according to your skin, the concern being treated and your response to the mandatory patch test.

What does non-ablative laser resurfacing mean?

Ablative lasers deliberately remove layers from the surface of the skin. They can provide significant resurfacing but are generally associated with more intensive healing and downtime.

The Palomar Icon 1540 is non-ablative. It delivers fractional heat below the skin’s surface without removing the entire outer layer.

This does not mean that the treatment produces no reaction or requires no recovery. Redness, swelling, sensitivity and temporary textural changes can still occur. However, recovery is generally less intensive than with fully ablative resurfacing.

Non-ablative treatment also works progressively. A course is usually required because each session stimulates a controlled repair response rather than attempting to achieve the entire correction at once.

Laser resurfacing for acne scars

Acne scars can take several forms. Some create shallow indentations, while others are narrow and deep, raised or associated with pigmentation and redness.

Palomar Icon 1540 laser resurfacing may help soften certain indented acne scars by stimulating collagen remodelling beneath the skin. As the tissue gradually reorganises, the depth and edges of suitable scars may become less noticeable.

Laser resurfacing will not treat every scar equally, and it is not intended for active, inflamed acne. Deep ice-pick scars, raised scars or scars attached to deeper tissue may require a different treatment or combination plan.

Your practitioner will assess the scar type before recommending treatment rather than applying the same procedure to every form of acne scarring.

Laser resurfacing for fine lines and skin texture

Fine lines do not all have the same cause.

Some develop because of repeated facial movement, while others become more visible as the skin loses collagen, firmness and support. Laser skin resurfacing is intended to improve skin quality and textural lines through collagen remodelling. It does not relax facial muscles or restore lost facial volume.

This means it should not be presented as a replacement for Botox, fillers or other injectable treatments. They address different concerns.

Laser resurfacing may be particularly valuable when the main goal is smoother texture, softer fine lines and healthier-looking skin rather than changing facial movement or structure.

Laser resurfacing for stretch marks

Stretch marks develop when the skin stretches or changes more quickly than its supporting structure can comfortably adapt.

The Palomar Icon 1540 may help improve the texture and appearance of suitable stretch marks by stimulating a controlled repair response within the affected tissue.

Older, pale stretch marks and newer red or purple stretch marks may respond differently. Several treatments are usually required, and complete removal cannot be guaranteed.

Your practitioner will assess the age, colour, location and extent of the stretch marks before advising whether laser resurfacing is likely to provide a worthwhile improvement.

The term “laser resurfacing” covers several different technologies, treatment depths and levels of intensity. A non-ablative fractional treatment should not be planned in the same way as a CO2 laser or a surface-level facial treatment.

What happens during laser skin resurfacing?

Before treatment begins, your skin will be cleansed and you will be given protective eyewear.

Your practitioner will position the cooled Palomar Icon 1540 handpiece against the treatment area and deliver controlled fractional laser energy into the skin.

The handpiece is moved systematically across the selected area so that treatment is delivered evenly according to the plan created for your skin.

A session may take between 15 and 90 minutes, depending on the size and number of areas being treated.

Does laser skin resurfacing hurt?

The treatment can feel hot, with a sensation often described as a series of small pinpricks against the skin.

Contact cooling helps make the procedure more comfortable. Sensitivity varies depending on the treatment area, the settings used and your individual tolerance.

Topical numbing may be discussed where appropriate, particularly for sensitive areas or more intensive treatment.

How many treatments will I need?

A course of at least four treatments may be recommended, although the exact number depends on the concern, treatment area and desired level of improvement.

Deeper scars and established stretch marks may require more treatment than mild textural concerns or early fine lines.

Your progress will be monitored throughout the course so that further treatment is recommended only when it is likely to provide additional benefit.

When will I see the results?

Laser resurfacing results develop gradually because the treatment works by stimulating the skin’s repair and collagen-remodelling processes.

You may notice some early improvement as the skin recovers, but further changes can continue to develop over the following weeks and months.

Results should be assessed after the skin has had enough time to remodel rather than immediately after treatment.

How will my skin look after treatment?

Your skin is likely to appear red and feel warm or sensitive immediately afterwards. Swelling can also occur, particularly when treating areas around the eyes.

During recovery, the skin may feel dry, rough or slightly tight. Temporary darkening, bronzing or a grid-like pattern can also occur, depending on the treatment response.

Redness and swelling may settle within several days, although visible changes can occasionally last longer. Your practitioner will explain what to expect based on the treatment area and settings used.

How much downtime should I expect?

Many patients return to work within 24–48 hours, but this does not mean that every visible sign of treatment will have disappeared.

Redness, swelling, dryness or textural changes may remain for several days and occasionally longer. Treatment around the eyes can produce more noticeable swelling.

It is sensible to allow additional recovery time before an important event, particularly for your first session when you do not yet know how your skin will respond.

Is laser skin resurfacing suitable for every skin tone?

Fractional non-ablative laser resurfacing may be suitable for a range of skin tones, but it should never be assumed to be appropriate without an individual assessment.

Naturally darker or recently tanned skin may carry a greater risk of unwanted pigment changes following laser treatment. The treatment settings, depth and timing must therefore be selected carefully.

If the Palomar Icon 1540 is not appropriate for your skin, we will recommend an alternative rather than taking an unnecessary risk.

Do I need a patch test?

Yes. A patch test is required at least 48 hours before your first treatment.

This allows us to assess how your skin responds and helps your practitioner select appropriate treatment settings.

A new patch test may be necessary if your skin tone changes, you develop a tan, your medical circumstances change or there has been a significant gap in your course.

Can I have laser resurfacing if I am tanned?

No. Laser treatment should not be performed on recently tanned skin.

You should avoid natural tanning, sunbeds and artificial tan within the treatment area before your appointment. If your skin becomes tanned, treatment may need to be postponed and a new patch test completed.

Consistent broad-spectrum sun protection is also essential following treatment because the recovering skin may be more vulnerable to unwanted pigmentation.

Is laser resurfacing better than microneedling for acne scars?

Neither treatment is automatically better for every patient.

Microneedling and fractional laser resurfacing stimulate skin renewal in different ways. Suitability depends on your skin tone, scar type, sensitivity, previous treatment history and the level of downtime you can manage.

Some patients may benefit from one treatment, while others may require a carefully planned combination approach. Your practitioner will recommend the option most likely to help your particular scars safely.

Can laser resurfacing replace Botox or fillers?

No. These treatments work in different ways.

Laser skin resurfacing aims to improve skin texture, collagen quality, fine lines and suitable scars. Botox relaxes selected muscles responsible for expression lines, while dermal fillers are used to restore or alter volume.

Laser treatment may be more appropriate when skin quality and texture are the main concerns, but it should not be described as a direct replacement for injectable treatments.

Can laser resurfacing treat active acne?

Laser skin resurfacing is primarily intended to improve suitable scarring and texture after active acne has been brought under control.

Treating inflamed or infected skin may increase irritation and the risk of complications. If you still experience active breakouts, your practitioner may recommend addressing the acne first.

Can laser resurfacing be combined with other treatments?

It may be combined with other treatments when your practitioner considers the combination appropriate and safe.

How should I prepare for treatment?

Before laser skin resurfacing, you should:

  • Avoid tanning and deliberate sun exposure
  • Stop using artificial tan within the treatment area
  • Tell us about all medications, supplements and skincare products
  • Avoid irritating or aggressively exfoliating the skin
  • Inform us of previous cold sores, unusual scarring or healing problems
  • Tell us about any changes to your health since your consultation
  • Follow the personalised preparation instructions provided by your practitioner

Do not stop prescribed medication unless the healthcare professional responsible for your care has advised you to do so.

What aftercare will I need?

Keep the treated area clean and follow the skincare instructions provided by your practitioner.

You may be advised to temporarily avoid active skincare ingredients, exfoliation, hot baths, saunas, swimming, strenuous exercise and other treatments while the skin settles.

Do not pick, scrub or peel dry or rough areas. Protect the skin from sunlight and apply the recommended broad-spectrum sunscreen.

Contact the clinic if you experience an unexpected or worsening reaction.

What are the risks of non-ablative laser resurfacing?

Expected reactions can include redness, warmth, swelling, sensitivity, dryness and temporary changes in texture.

Less common complications include prolonged redness, blistering, crusting, infection, cold-sore reactivation, unwanted darkening or lightening of the skin and scarring.

Your individual risks will be discussed during your consultation. Following the preparation and aftercare instructions is an important part of reducing them.

Who will carry out my treatment?
